Community Project Worker

Part-Time | Fixed-Term | £15 per hour (25 hrs/week)

Caafi Health Care Limited is looking for a Community Project Worker to help people with their medicines after they leave hospital.

This job is part of the Tech for Better Care project, which is testing new digital tools to make life easier for people managing their medicines.

You will:

  • Visit people in their homes or hospital.

  • Show them how to use simple technology to take the right medicines at the right time.

  • Work with nurses, doctors, pharmacists and other community groups.

  • Keep clear notes and share updates with your manager.

Ideal candidates will:

  • Friendly, caring and good at talking with people.

  • Able to use computers, phones and simple digital tools.

  • Happy to travel around Bristol and Weston.

  • Able to work on your own and also as part of a team.

  • Ideally have a full driving licence and transport.

    It would be great if you also:

    • Have worked with people who have health or social care needs.

    • Have experience in a hospital or community health job.

    • Know about older people’s needs or social prescribing.

Care Support Worker

Caafi Health Care is looking for kind and reliable Care Support Workers to join our team. You will provide practical and emotional support to individuals and families, helping them to maintain independence, dignity, and wellbeing in their homes and community settings.

Key Responsibilities

  • Provide personal care and support

  • Assist with daily living activities such as meals, hygiene, and medication

  • Support service users with access to community services and health appointments

  • Promote wellbeing through companionship, encouragement, and culturally sensitive care

  • Keep accurate records and communicate effectively with the wider support team

What We’re Looking For

Essential:

  • Has own car

  • Able to communicate and write clearly in English

  • Must be eligible to work in the UK and pass a DBS check

Desirable:

  • Previous care experience

  • Completion of the Care Certificate
